the Ottawa Supportive Housing Network has been established to streamline access to housing for people living in the City of Ottawa who require accommodation and support. With this application form, you can be considered for housing offered by the agencies that are part of the Network.

Instructions

1. Download and print out the application form
2. Review the list of agencies that form the Network and decide which ones should receive your application.
3. Fill out the application form (print clearly in ink) and fax or mail a copy to each agency that you wish to receive your application (contact information is included in the list of agencies).
4. You will be contacted by these agencies according to their application procedures.
